Anchor Health Services is a home base healthcare provider through the Minnesota Department of Human Services, with a targeted mission of providing comprehensive, Inclusive, and compassionate healthcare services based on a community’s unique needs across the twin cities. Anchor Health Services take both individual and community needs of love, care, and respect in our healthcare deliveries.

Our registered nurses will follow established agency policies and procedures, federal and state regulations, and nursing standards of practice. They will perform physical assessments, track and report results to the client’s physicians, as well as provide other case management services. They will partner with other medical professionals and our client’s physicians to ensure that all client’s health needs are met. Our registered nurses will also provide medication setup, refill and monitor the effectiveness of the medication as required by our client’s physician and keep the physician up-to-date on the progress of the client. 

Our licensed practical nurses assist our registered nurses in providing total client care following established agency policies and procedures, federal and state regulations, and nursing standards of practice. They perform any or all professional nursing duties as determined by qualifications and training and perform non-professional nursing duties as necessary. Our licensed practical nurses also provide medication setups, refill and monitor the effectiveness of medications as required by our client’s physician and keep the physician up-to-date on progress.
